SERVICE AGREEMENT INFORMATION This agreement states that Good Stay, LLC employees may enter your house to take care of pets in exchange for a monetary amount. We require a 72 hour notice on all holiday cancellations and a 24 hour notice for all other. Failure to do so will face the cost of the service booking. Pick up and check out times for boarding are as follows: 8AM - 10AM at no extra charge 10AM - 8PM at $20 weekdays will include an off-leash hour and a drop off 8PM - 10PM at $30 Please call ahead for an appointment Rates can change but will not without one month warning. WE DO NOT accept credit cards at this time. (I'd have to jack the rates up if I did). We accept cash or check. Please make checks out to Good Stay. Good Stay, LLC is insured against any negligent act preformed by Good Stay, LLC. So if your dog is injured playing, you're picking up the tab. If your dog slips the lead and runs into traffic, we pick up the tab. In the event of an emergency room visit we will use all available contacts to try to get hold of you unless you clicked on the radio button located in the contact information, asking us not to disturb you. Leave the next agreement blank if you have no monetary limit to treating your pet.

I agree to pay the following amount if my dog is injured:   $

Vaccinations, Licensing and Health related issues Owner will provide vaccination documentation one week prior to first boarding date including: rabies, distemper, hepatitus, leptospirosis, para influenza, parovirus and bordatella (kennel cough) Your dog should be free of communicable diseases for 30 days prior to staying. We do not accept aggressive dogs, toy or food. Well the food one you might be able to talk us out of that. If your dog will be with us at off leash areas: They must be licensed, play well with others, and stick with us (Chet, can you hear me typing this?) Good Stay reserves the right stop service at anytime. The fore mentioned reasons are why certain dogs like Chet don't make it. We need to be able to recall your dog in the event of a bonehead showing up at the park with a dangerous dog. Your dog will be allowed to stay kennel free if: It's house broken Can chill out when other dogs would like get some rest It won't tear up the furniture Rates are currently: $20 round trip for a park run day care $30 for boarding which includes a park run day care Rates are all inclusive of tax and what not. Boarding will go up to $40 after the back yard gets a remodel in the summer of 2007
